The Case of the Sad Doctor

A 55-year-old dentist begins to experience changes in mood. He is losing interest in his business and lacks the desire to play his daily tennis match. He begins to have difficulty thinking and concentrating, and lacks decisiveness. He is preoccupied with feelings of guilt, worthlessness, hopelessness, and paranoia. He has come to you in search of psychological help. He tells you that he has been feeling this way for about a month, but can’t understand why, as there have been no significant negative events in his life he can recall.

He tells you that he has considered ending his life on occasion, but doesn’t feel he would ever follow through with any serious attempts because of all of his responsibilities.

In addition to the symptoms stated above, the patient lists the following physical complaints: severe reduction in appetite, insomnia, reduced sexual drive, weight loss, and constipation. He states he feels his body is rotting away with cancer.
